Some important item analysis data to interpret pertains to the reported incorrect answers. Incorrect answers that have been chosen by a significant percentage of students, require examination for what might be distracting them to the wrong answer, for concepts or content that are confusing, or for vocabulary, formats, and/or processes that might be creating a problem.

Red flagging requires adding another column. Notations are made in this column as to which incorrect answer is seems to be causing a problem. Those doing this analysis also need to refer to the test questions themselves to help determine where the difficulties might exist.
